The below information is available to support you through the various phases of the injury management process.

Making a claim for a workplace injury

If you're injured at work, it's important to tell your employer immediately. They should provide first aid if it's appropriate, and help you seek medical treatment.

Your employer must notify their claims service provider of the injury or significant illness within 48 hours.

In the event that your employer hasn't notified their claims service provider of your injury, you or your representative (eg. family member, doctor) may notify them on your behalf. If you have any questions or are unsure which claims service provider to lodge your claim with, you or your representative can contact us or ask your employer.

If you've had time off work and/or medical treatment, we will contact you, your employer, and if necessary, your doctor to determine how best to support your recovery.

The following information is required when you notify DXC of an injury:

  • your name, address, contact telephone number and date of birth
  • name and address of the company where you work
  • date and time of the injury and description of the injury and details of how it happened
  • name and contact details of someone where you work who will be able to discuss your injury (for example human resources (HR) representative or business owner)
  • date when you stopped work
  • if you had time off work, and if medical treatment was required
Getting a Certificate of Capacity

A Certificate of Capacity is the primary communication tool between you and your medical support team.

Having it will ensure your recovery needs are communicated accurately to the rest of their support team, including your employer, your case manager and any other key stakeholders.

Completing the Certificate of Capacity will also help ensure:

  • The DXC team is able to assess your worker's claim
  • You are able to receive compensation payments
  • You have a clear plan for treatment, rehabilitation and guidance to return to work

Medical expense reimbursements

You may be entitled to reimbursement of medical and like expenses related to your injury. Click for details on claiming treatment expenses.

Submit your medical and like invoices for reimbursement to DCM-Invoices@dxc.com

Important notes:

  • Ensure the invoice/receipt is clear and all information can be clearly read (e.g. that an attached receipt doesn’t cover any invoice information)
  • Always include your claim number when submitting any invoices
